What can I expect at my first Neurology visit?
You will be greeted by our Multi-Specialty Clinic staff upon arrival to a neurology consult. Our staff will collect your photo ID and insurance information for registration. The Neurologist will then complete your consultation.

How can I schedule an appointment?
You may schedule an appointment with Dr. Afzal or Dr. Srivastava by calling 815-226-1906. They alternate visits in Rochelle, seeing patients every Tuesday and Wednesday at the Rochelle Multi-Specialty Clinic from 1:00 p.m. – 5:00 p.m. Neither doctor requires a physician referral, but check with your insurance to determine if one is needed for you. Your medical records will need to be forwarded to the office before your first appointment.
