Patients & Visitors Hospital Patient Portal How Do I Enroll?

Before you begin, you will need the following information on record at Rochelle Community Hospital (RCH):
- Patient's Name as it appears in the system
- Patient's Medical Record Number or last 4 digits of Social Security Number
- Date of Birth
- Email Address on file with RCH
This information will be provided if you register for outpatient testing or procedures or upon discharge from emergency services/inpatient services. If you have any questions, please contact our Health Information Management Department at 815-562-2181, ext. 2740.
To access , you must know your Medical Record Number or last 4 digits of your Social Security Number and have an email address on file with Health Information Management. If you do NOT have an email address on file with Health Information Management, please click on the button below before proceeding with enrollment.
Self-Enrollment
Once you have secured the needed information above, you will be able to register an account on .

You will see this screen. Fill in the necessary fields to begin.
Once the information has been entered, click on the SUBMIT tab. Then you should see the following screen:
You will need to access your email with subject line "Patient Portal Password." Click on the URL address within the email. You will be launched directly into .
Create a logon ID that is more than 8 characters including alpha and numeric characters.
You will also need to create a password and set up a security question and answer.
Once you have set this up, click SUBMIT. Log off . Next, you will need to log back in using your new logon ID and password you have created.
You will then be prompted to accept the terms and conditions of . Once you click ACCEPT, you will have access to your Health Portal.
