Welcome to the Rochelle Community Hospital Health Portal.
This interactive web portal empowers our patients and their families to take a more active role in their care by providing easy, secure access to their health information and online communication with our staff.
If you are interested in enrolling in the Health Portal, you must visit our Health Information Management (HIM) department at the hospital to begin the enrollment process which requires your signature for appropriate authorization to gain access to your health information via the Health Portal.
Once a patient has enrolled in the Health Portal, you will be allowed instant access to your health information, including laboratory and radiology results, medications, allergies, demographic information, visit history, discharge/aftercare information and more. In addition, parent and healthcare proxies with authorized consent will appreciate the ability to access the records of those individuals for whom they manage care.
The Health Portal allows patients to view their appointments online. A central appointment page displays their list of scheduled and pending appointments. Here, they can review appointment details.

Notice of Data Practices
In our ongoing efforts to improve the quality of care at Rochelle Community Hospital, we are offering a new way to communicate with us online. The Health Portal is a secure, web-based system that allows real time access to certain medical record functionality:
- Reviewing your personal health information
- Updating your profile and contact information
- Reviewing billing statements
When you connect to Health Portal, you are NOT connecting directly to our hospital medical records system; Health Portal is a secure and separately maintained service. Only the limited information you see within the service is stored on Health Portal, and your actual medical records are maintained in our hospital.
In order to use Health Portal, you must first obtain a username and password. For security reasons, we cannot register patients to use Health Portal over the phone. To use Health Portal, you must maintain an email address that you check regularly. You will be asked for your email address upon registration at our hospital. Private health information will not be sent to your email; instead the email will alert you that you have a new message posted on Health Portal. We want to keep your health information secure and confidential.
Important Information About Health Portal:
- Use is for non-emergency communication.
- We will not send any private health information to your email.
- We will send you an email only when necessary, to request that you access Health Portal to review private healthcare information we have posted in your Health Portal account.
- Documents and forms cannot be attached to Health Portal messages.
- Health Portal is not checked on the weekends. It is only checked during normal business hours.
Health Portal Data Practices:
Our privacy obligations, and your rights as a patient, are described in our Notice of Privacy Practices. You may obtain a copy of our Notice of Privacy Practices, or at any of our treatment locations. We will not use or disclose the information in Health Portal except as provided in our Notice of Privacy Practices.
Consistent with our Notice of Privacy Practices, we may release Health Portal information for reporting about our hospital and patients. For example, we may release information to auditors or for public health activities as further described in our Notice of Privacy Practices. We may also release Health Portal information for the purpose of developing software applications associated with the operation and improvement of Health Portal. Except as specifically provided in our Notice of Privacy Practices, and consistent with applicable law, we do not release Health Portal information for any of the following purposes: (i) marketing or advertising; (ii) medical or pharmaceutical research; or (iii) to your insurer or employer.
When required by applicable law, we require Limiting Agreements that restrict what third parties can do with your Health Portal information. Limiting Agreements are legally binding agreements that prohibit certain third parties from releasing your information or identifying specific individuals.
We stop releasing your Health Portal information if you close your Health Portal account. However, we will continue to maintain, use and disclose information from your actual medical record as provided in our Notice of Privacy Practices, and in accordance with all applicable laws and regulations.
We have security measures that are reasonable and appropriate to protect Health Portal information from unauthorized access, disclosure or use. We store all Health Portal information in the United States only. We keep Health Portal activity logs, which are available for your review upon request.
Health Information Exchange
In addition to the Health Portal, you should be aware that we participate in the electronic exchange of health information. This technology allows a provider or a health plan to make a single request through a health information exchange (HIE) to obtain electronic records for a specific patient from other HIE participants for purposes of treatment, payment or healthcare operations. HIEs are required to use appropriate safeguards to prevent unauthorized uses and disclosures.
If you wish to make your medical information available through the HIE, you do not have to do anything. You may opt-out of participation in the HIE by completing a form. If you opt out of HIE participation, your decision will apply to all of your medical information. Due to technical limitations, you cannot include some portions of your medical information in the HIE but exclude other portions.
