Rochelle Community Hospital has taken every precaution to ensure electronic health records are secure, confidential and respectful of your privacy.
May I allow someone else to have access to my health records?
Yes. This is called "Proxy Access" and you should be aware of the following: A parent or legal guardian for a child under 12 may access the child's account and view the record.
However, per State and Federal guidelines, after the child turns 12, parents/guardians are now required to have an additional authorization signed by the child to gain access to that child's personal medical records.
Once a child turns 18, parents/guardians can only access that child's personal medical records with his/her authorization.
If you help manage an adult's medical care, it is possible to request access to that person's health record. For more information, please contact the Health Information Management Department at 815-562-2181, ext. 2740.
Who else may access my health information at Rochelle Community Hospital?
Only the patient and the individuals the patient has authorized as proxy representatives.
How much does this cost?
It is free of charge to every Rochelle Community Hospital patient.
Some of my information is incorrect. How do I correct this?
If you notice an error in your personal information, such as address, phone number or email address, please send us a change request through the portal by clicking "Profile" and then "Update Profile."
If your medical history is incorrect, please notify the Health Information Management Department at 815-562-2181, ext. 2740.
How do I see my test results?
For laboratory or radiology results, start by clicking the "Health Record" button and then selecting the appropriate button on the right hand side of the screen:
- Laboratory - check "Results"; these will be available 12 hours after resulted.
- Radiology - check "Reports"; these will be available 24 hours after resulted.
May I change or update my medications and/or allergies listed in?
Your medications and/or allergies can be changed or updated the next time you are admitted either as an inpatient or emergency room patient. The medications and allergies listed are updated and reviewed at each visit.
Can I participate if I don't have an email address?
No. To use , you must have an email address.
